If this is your first time coming to this location, new client forms must be filled out.

Attached below to print and fill out before first appointment or please arrive 10 minutes early to your appointment to fill them out then.

No-Show/

Cancellation Policy


Failure to cancel your appointments at least 24 hours in advance will result in a charge of 50%. Failure to show up to your appointment or same day cancellations will result in a charge of 100% of the scheduled appointment fee which will be processed on the credit card retained on file to reserve the appointment at booking or sent via invoice.

If you need to reschedule because you are not feeling well please contact me as soon as possible to reschedule. Thank you

COVID-19

WHAT IS BEING DONE TO PROTECT YOU:

-Therapist has been vaccinated in all vaccines necessary to ensure the safety of everyone.

Scheduling additional time between each client to thoroughly sanitize and deep clean treatment room and massage equipment with high quality disinfectants.

– Staggering all appointment times to minimize social contact in common areas of the center and maintain social distancing.

-If asked Therapist is willing to wear a mask during the session, please do not hesitate to ask if that makes you feel more comfortable.

– As always, linens are changed between clients. Table with extra cover is also wiped down and disinfected.

– Disinfecting high-touch surfaces, common areas, and restrooms regularly throughout the day.
